Advanced Stem Cell Therapy for Liver Failure

Every day, R3 Stem Cell receives inquiries worldwide from individuals asking, “Can stem cell therapy help with liver failure?” Spoiler alert: It can help a lot! In this guide, we’ll walk you through the fundamentals of stem cell therapy for the liver, the latest research, and what to expect from a regenerative procedure.

Why the Liver’s Regeneration Ability Makes It Unique

Unlike having two kidneys, humans are only blessed with one liver. This organ isn’t just the heaviest in the body—it’s also the only one capable of regenerating itself. Amazingly, if up to 70% of the liver is removed, the remaining tissue can regrow.

Because of this remarkable regenerative ability, stem cells hold tremendous potential to stimulate and support the liver’s repair. Much like a lizard regrows its tail, stem cell therapy can help regenerate diseased or non-functioning parts of the liver, restoring crucial organ function.

A Significant Global Health Issue

The Burden of Chronic Liver Disease

Chronic liver diseases (CLD) are a major health and economic concern worldwide. Over 800 million people suffer from chronic liver diseases globally, and 2 million deaths are recorded every year from liver-related complications.

Cirrhosis: The Final Stage of Chronic Liver Disease

Cirrhosis represents the end-stage of chronic liver disease (CLD). It often remains asymptomatic for a long time, which delays diagnosis until the disease has progressed significantly. Because of this, chronic liver disease has become the 14th leading cause of death among adults worldwide.

Causes of Cirrhosis

Several factors contribute to chronic liver damage and eventually lead to cirrhosis, including:

Other Liver Pathologies That Can Lead to Cirrhosis

Less common liver conditions may also cause end-stage liver disease, such as:

As liver fibrosis progresses, the liver gradually loses its ability to function, leading to cirrhosis and, in many cases, liver cancer.

Limitations of Traditional Therapies for Liver Disease

Symptom Management, Not a Cure

Traditional treatments for end-stage liver disease focus mainly on managing symptoms rather than addressing the root problem. Liver transplantation is the only definitive solution for many patients with cirrhosis, but it comes with several challenges:

Access Challenges for Transplantation

Due to economic and social limitations, only a small percentage of patients who need a transplant receive one. Patients over 70 years old are often excluded from transplant eligibility, making it even more difficult for some to access life-saving treatments.

What Does the Liver Do?

The liver plays a vital role in maintaining health, making it crucial to keep it functioning properly. It performs several critical functions, such as storing, producing, detoxifying, and regulating substances within the body. Below are the key functions of the liver:

What are the reasons a liver might fail?

The liver can fail for several reasons, some of which are preventable, while others are linked to genetic disorders. Chronic alcohol abuse is one of the leading causes of liver failure worldwide. Additionally, misusing prescription medications or herbal compounds can lead to liver damage.

How Liver Failure Progresses

Liver failure doesn’t happen overnight. It usually follows a predictable pattern involving several stages:

Chronic Inflammation

Like other chronic diseases, liver conditions begin with persistent inflammation, damaging healthy cells over time.

Fibrosis

As inflammation continues, the liver develops scar tissue (fibrosis), making it harder for the organ to perform its essential functions.

Cirrhosis

When fibrosis becomes extensive, it leads to cirrhosis—a condition where large portions of the liver become non-functional.

End-Stage Liver Failure

As cirrhosis worsens, the liver reaches end-stage failure, requiring advanced medical intervention, such as stem cell therapy or transplantation.

What Are the Symptoms of Liver Failure?

Long-term liver injury gradually leads to liver dysfunction, causing an accumulation of extracellular matrix (ECM) and resulting in liver fibrosis. The final stage of fibrosis is cirrhosis, where liver function deteriorates.

Patients with decompensated cirrhosis experience severe complications, such as ascites (fluid accumulation in the abdomen) and gastroesophageal variceal bleeding (bleeding from swollen veins in the esophagus).

As cirrhosis progresses, late-stage complications can develop, including:

Without a liver transplant, many patients with these advanced symptoms do not survive beyond two years, as traditional therapies only manage symptoms without addressing the root cause.

Challenges of Traditional Liver Failure Treatments

Conventional therapies for liver failure are limited and primarily focus on symptom management. For alcohol-related liver disease, the first step is to stop alcohol consumption—a process that must be supervised by a healthcare provider. Patients with obesity are advised to lose weight through a balanced diet and exercise to reduce liver strain.

While certain medications, such as blood pressure drugs or diuretics, may be prescribed to manage complications, there are no medications currently available that can reverse liver damage or restore liver function.

For patients with kidney failure, dialysis can substitute kidney function, but no such machine exists to replicate the functions of the liver. In cases of portal hypertension, a TIPS procedure (Transjugular Intrahepatic Portosystemic Shunt) can be performed. This creates new connections between liver blood vessels, relieving pressure and reducing symptoms.

At R3 Stem Cell, we have seen several patients successfully avoid liver transplants by combining TIPS procedures with our regenerative therapies, restoring enough liver function to be removed from transplant lists.

Liver Transplantation: The Last Resort

Patients with end-stage liver failure are often placed on a transplant list, but this process comes with many challenges. A liver transplant in the USA can cost over $800,000 USD, with average wait times of 240 days.

Globally, eligibility for transplantation is limited by age and health conditions. It’s rare for patients over the age of 70 to be considered for a transplant, and many others face exclusions due to medical or social circumstances.

Stem Cell Treatment for Chronic Liver Diseases

If a new technology like mesenchymal stem cell (MSC) therapy or exosome therapy could reverse liver failure or slow its progression, it would rightly become the first-line treatment. Since the liver has a unique ability to regenerate, regenerative therapies offer a promising option to restore function safely.

While liver transplantation remains viable for many patients worldwide, it comes with several limitations, including:

These limitations highlight the urgent need for alternative therapies. Recent research demonstrates that mesenchymal stem cells (MSCs) are an effective treatment option for repairing liver injury and promoting tissue regeneration.

How Stem Cells and Exosomes Work

Stem cells and exosomes exert their therapeutic effects through several key mechanisms. Notably, they do not alter a patient’s DNA or become part of the body’s existing cells. Instead, they act through the following processes:

Angiogenesis

Promotes the formation of new blood vessels, improving circulation.

Reducing inflammation

Chronic liver diseases are characterized by significant inflammation, which these biologics help mitigate.

Immune system modulation

Unlike steroids, which suppress the immune system indiscriminately, stem cells and exosomes regulate immune responses selectively. They suppress harmful processes while boosting beneficial ones, including the production of growth factors and cytokines.

Cellular signaling

These therapies enable cell-to-cell communication, encouraging the body’s cells to produce proteins and regenerate damaged tissues

Preventing apoptosis

Apoptosis is the process of programmed cell death. Stem cells help healthy liver cells (hepatocytes) survive longer, delaying premature cell death.

Anti-fibrosis

Liver failure often results in extensive scarring (fibrosis). Stem cells and exosomes reduce scar tissue formation, helping to maintain liver functionality.

MSC-Driven Tissue Repair and Regeneration

MSCs also release a wide variety of molecules into the extracellular environment. These include:

These molecules play critical roles in repairing damaged tissue and promoting regeneration. In addition to offering MSCs, R3 Stem Cell includes stem cell-derived exosomes as part of the treatment. Exosomes act as specialized vesicles that facilitate cell-to-cell communication, supporting liver tissue repair and regeneration.

Is there research to back up stem cell and exosome therapy for liver disease?

Numerous clinical trials worldwide have demonstrated outstanding outcomes for stem cell therapy in treating liver failure.

In a study involving 45 chronic hepatitis B patients with decompensated liver cirrhosis (LC), Umbilical Cord Mesenchymal Stem Cell (UC-MSC) transfusions resulted in a significant reduction in ascites volume compared to the control group. The therapy also improved liver function, evidenced by:

Another trial, conducted in China in 2012, studied 43 acute-on-chronic liver failure (ACLF) patients with HBV (Hepatitis B Virus) infection. Patients treated with UC-MSC transfusions showed the following improvements:

Between 2010 and 2013, Bing-Liang et al conducted an open-label, non-blinded randomized controlled study on 110 patients with HBV-related ACLF, published in the Journal of Translational Medicine. The experimental group of 56 patients received one million stem cells per kilogram of allogeneic MSCs weekly for four weeks, with follow-ups over 24 weeks. This group showed:

The largest long-term study of its kind, led by Zheng et al, followed 219 patients with HBV-related decompensated liver cirrhosis in a prospective, open-labeled randomized controlled study. Participants were split into:

Patients in the treatment group received three UC-MSC infusions at four-week intervals, alongside conventional therapies administered to both groups. After seven years of follow-up, the UC-MSC-treated group showed sustained improvements in liver function, demonstrating the long-term effectiveness of stem cell therapy.

Stem Cell Derived Exosomes

R3 Stem Cell’s Centers of Excellence globally incorporate umbilical cord stem cell-derived exosomes with umbilical cord stem cells to enhance therapeutic outcomes. Exosomes are lipid-bound vesicles (acellular) that contain growth factors, cytokines, mRNA, and other proteins.

These exosomes play a critical role in cell-to-cell communication and are particularly effective in reducing inflammation once they are absorbed by recipient cells. They function as shuttles, transporting nucleic acids and proteins between cells, thus facilitating communication among both local and distant cells. The proteins released by exosomes act as key regulators of intracellular processes.

Exosomes are likely responsible for many therapeutic effects associated with stem cell therapy. Being 100 times smaller than stem cells, they can easily pass through the lungs without issue, traveling efficiently to the liver via the bloodstream.

Protocol

For over a decade, R3 Stem Cell has successfully treated liver failure patients with IV stem cell and exosome therapy. These biologics naturally target inflammation, which is a significant component of liver failure, concentrating their therapeutic effect primarily in the liver. They also migrate to other areas of the body experiencing disease.

Some people ask us, “I heard the stem cells get caught in the lungs and die, is that true?” The answer is mesenchymal stem cells do often temporarily get caught up in the lungs, but they don’t all stay there. After 12-24 hours, the vast majority are released to go to areas with inflammation, such as a failing liver.

So, for example, if a person has liver failure secondary to diabetes, the cells and exosomes will also go to the pancreas to assist with function there too. There are Centers that promote injections directly into the liver, or hepatic artery/vein. This is not necessary and entails additional risk!

R3’s providers will calculate the amount of stem cells based on patient weight and liver failure severity. It will range from 1 to 3 million stem cells/kg. Depending on the total amount, treatment may need to be broken up into two sessions, three at the most for optimal safety.

R3 Stem Cell’s liver disease protocols are based on the latest research along with Best Practice Protocols developed over the past decade to help patients achieve the best outcomes possible. Safety is paramount with the biologics products being rigorously tested prior to use, and expert providers managing each treatment as if it were a family member!

Outcomes

R3 Stem Cell’s results for liver failure patients have been exceptional, with a patient satisfaction rate of 85% year-over-year. Patients commonly report increased energy, improved cognitive function, reduced ascites, and enhanced liver function.

In line with clinical research, most patients experience improvements in liver function tests, including higher albumin levels, increased cholinesterase and prothrombin activity, and higher platelet counts. Additionally, serum total bilirubin and alanine aminotransferase levels decrease significantly following UC-MSC transfusions.

Although it may take several months to see all the improvements, many patients begin to feel better within a few weeks. It’s important to note that stem cell therapy is not a cure for chronic liver disease and requires repeat treatments every 6 to 12 months to maintain benefits.

Affordability

Because stem cell therapy for liver failure is not a permanent cure, affordability is essential. Repeat therapies help maintain progress or achieve further improvements. Many patients return to R3 Stem Cell for additional treatments every 6 to 18 months.

In comparison, stem cell clinics in Colombia, China, and Panama charge over $20,000 USD per treatment. This makes it challenging for patients to afford yearly sessions. R3 Stem Cell offers the same high-quality care at less than half that cost for 100 million premium stem cells.

R3’s Experience

Over the past decade, R3 Stem Cell Centers have performed more than 23,000 regenerative procedures across six countries, with several hundred treatments for liver failure. Across all conditions, patient satisfaction stands at 85%.
