Amniotic Allograft for Ophthalmology and Optometry

 

Amniotic membrane (AM) has been used for decades in the field of eye care. This includes being used as either a biologic ocular bandage to assist healing or as an ocular transplantation graft to repair or replace damaged tissue.

 

Traditionally, AM was placed in the operating room using either fibrin glue or sutures. However, new studies are showing the effectiveness of placing AM as an outpatient in an office setting for such conditions as various types of keratitis, corneal ulcers, neurotrophic keratopathy, and chemical burns.

 

This is opening up treatment options with AM to many more eye doctors for the office setting. It is also providing much more effective treatment for various conditions than what currently exists. Conditions include:

 

As a surgical graft:

 

  • Pterygium and pinguecula surgery
  • Conjunctivochalasis and conjunctival lesion surgery
  • Superior limbic keratoconjunctivitis
  • Leaking bleb repair
  • Descemetocele or perforation
  • Fornix and socket reconstruction

 

As a biologic bandage:

 

  • Recurrent corneal erosion and post-superficial keratectomy
  • Corneal ulcers
  • Bullous and band keratopathy
  • Chemical burns
  • Stevens-Johnson Syndrome
  • Partial limbal stem cell deficiency
  • persistent epithelial defect (PED) of the cornea

 

The use of amniotic tissue for assistance with wound healing has been advocated for over 65 years. Amniotic membrane tissue is indicated for the management of ocular wounds.

 

The allograft maintains a moist environment which helps provide a physiologically favorable milieu for ocular wound management such as in the reduction of inflammation and scarring, and in reducing abnormal blood vessel growth. Amniotic membrane transplantation has proven beneficial where frequent topical lubrication has failed and as an alternative to tarsorrhaphy, punctal occlusion, tissue adhesives, and conjunctival flaps.

 

In addition, the use of amniotic membrane for treating dry eye is not a novel concept. A 2015 study out of Nepal evaluated amniotic membrane transplantation in fourteen eyes. The results were excellent, with significant reduction of inflammation and symptomatic relief from dry eye. One thing the study noted was they simply applied the AMT as an overlay without sutures or fibrin glue.
